Northwest Permanente, P.C. is currently seeking Board Certified or Eligible Internist, Family Practitioner, Geriatrician or Hospitalist to join our dedicated team of Skilled Nursing Facility Physicians and growing medical group. We are growing our team in Salem, OR. Salem, Oregon is the state capital and has approximately 155,000 people. The service area within Northwest Permanente has approximately 50,000 members. Among the many systems which support our daily practice is an award-winning Windows-based electronic medical record system which allows for easy access to all pertinent patient clinical data.
You will work closely with an experienced, cohesive interdisciplinary team. This includes nurses, therapists and social workers that provide after-hospital care in the SNF setting. We emphasize transitions of care from the hospital to the skilled nursing setting and from a skilled nursing facility to home or other settings of custodial care. We provide a team approach, usually with two 'SNFists' and a Kaiser Care Coordinator at each facility, that care for a spectrum of medical complexities, ranging from straight-forward, elective orthopedic surgery patients to those with multiple, complex medical problems.
Department goals include decreasing emergency room visits and re-hospitalization, as well as transitioning to palliative and hospice care, when appropriate. You will enjoy a predominantly weekday, daytime work schedule. Our growing department shares evening home-call and weekend admitting responsibilities. We also feature a robust, Epic HealthConnect-based charting.
