Tobacco Cessation Outreach Coordinator job in Medford

AllCare Health is seeking a Tobacco Cessation Outreach Coordinator for our Medford Office who wants an exciting opportunity to educate and make a difference in our community with our health plan members.
AllCare Health is a southern Oregon-based health organization, led by local physicians since 1994. In a progressive model of care, we combine medical, dental, and behavioral health services to provide Medicare, and Medicaid insurance coverage to our members. We also provide support to our providers through AllCare Independent Physician Association and AllCare e-Health Services

The Tobacco Cessation Outreach Coordinator is the key contact and conducts weekly support groups and provides phone support to those who have the desire or need to quit smoking for CCO, and Medicare Advantage health plan members. Tobacco cessation counseling services and outreach include face to face, small group, phone and community event outreach material and events.

Essential Duties:
 Oversee the assessment, development, intervention(s) and evaluation of the tobacco cessation program development, activities and projects for our members.
 Work with members with special needs, pregnant, current/history of mental and/or drug and alcohol issues.
 Participate in offered trainings regarding motivational interviewing, communication skills.
 Participate and work with other staff on coordination of care, processes and represent tobacco cessation efforts as part of the Interdisciplinary Care Team.
 Participate in community partner meetings, committees, projects, and events as appropriate for collaboration, outreach and education regarding tobacco cessation.

Qualifications:
 Current facilitator training in smoking cessation quit for life and freedom from tobacco preferred.
 Two years previous experience to include: tobacco cessation outreach and current product knowledge.
 Working knowledge of the Oregon Health Plan and Medicare.
 Computer competency working in a windows environment and Microsoft Office.
 Knowledge and skill of excel spreadsheets desired.
