Intensive Case Manager job in Medford

AllCare Health is seeking a Full-Time Intensive Case Manager (ICM) for our Medford Office who thrives in a team environment and has the ability to provide exceptional service and skills.
AllCare Health is a southern Oregon-based health organization, led by local physicians since 1994. In a progressive model of care, we combine medical, dental, and behavioral health services to provide Medicare, Medicaid insurance coverage to our members. We also provide support to our providers through AllCare Independent Physician Association and AllCare e-Health Services

The ICM is a resource person that serves AllCare Health members with special health care needs.

Qualifications:
 Experience in health and social service management required.
 Advanced communication and interpersonal skills with all levels of internal and external customers, which includes promotion of positive public relations.
 Computer competency working in a windows environment and Microsoft Office

Essential Duties:
 Provides education to members and contracted providers.
 Collaborates with community based organizations in order to ensure the members are connected with the appropriate available assistance.
 Participates in all Transition of Care and Interdisciplinary Care Team meetings and activities.
 Represents AllCare Health in community activities that promote community service partnerships.
