Community Health Worker job in Grants Pass

AllCare Health is seeking a Community Health Worker who thrives in a team environment and has the ability to provide exceptional service and skills.
AllCare Health is a southern Oregon-based health organization, led by local physicians since 1994. In a progressive model of care, we combine medical, dental, and behavioral health services to provide Medicare, and Medicaid insurance coverage to our members. We also provide support to our providers through AllCare Independent Physician Association and AllCare e-Health Services

The Community Health Worker works collaboratively with the Primary Care Provider (PCP) Team to plan for and address the case management needs of individual members at risk of high utilization of Emergency Department services and/or avoidable hospitalizations due to co-morbid conditions including, but not limited to, chronic diseases, mental health, and/or chemical dependency diagnoses and related social determinants of health.

Qualifications:
 High School Diploma or equivalent (GED)
 Current experience working in the community, preferably on health issues
 Valid driver’s license, reliable transportation, and proof of automobile insurance
 Must have experience with computer data entry, word processing, excel and typing
 Interested applicants with bi-lingual proficiency are encouraged to apply
 Basic medical knowledge including ability to recognize social and medical risk factors, trauma symptoms and behaviors, and chronic disease conditions
 Cultural knowledge and sensitivity to the population being served
 Medical terminology
 Knowledge of community resources
 Ability to work independently and as part of a team

Essential Duties:
The Community Health Worker provides information, assistance, tools, and support to enable a member to make the best health care decisions in the member’s particular circumstances and in light of the member’s needs, lifestyle, combination of conditions, and desired outcomes. Members are seen in PCP offices, their homes or similar environments.
