AllCare Health is seeking a Full-Time Chronic Care Disease Manager who thrives in a team environment and has the ability to provide exceptional service and skills.
AllCare Health is a southern Oregon-based health organization, led by local physicians since 1994. In a progressive model of care, we combine medical, dental, and behavioral health services to provide Medicare, and Medicaid insurance coverage to our members. We also provide support to our providers through AllCare Independent Physician Association and AllCare e-Health Services
The Chronic Care Disease Manager is responsible for providing disease education evidence-based wellness promotion and self-care management solutions to our members of all ages who have chronic diseases and/or conditions. The process includes assessment, planning, implementation, coordination and required to meet member's health care needs.
Qualifications:
Minimum two years previous experience to include: Care Coordination, Disease Management, Managed Care or Homecare Coordination
Current RN or other applicable health professional licensure in the state where they work free of any restrictions of disciplinary action pending and able to legally and independently practice without the supervision of another licensed professional
CCM certification preferred or successfully completing the certification within two years of employment
Three years clinical experience with preferred experience in conditions they will be managing
Computer competency working in a windows environment and Microsoft Office
Essential Duties:
The CC Disease Manager will act as a member advocate and facilitate collaboration with community Providers and the health care team; to assist members to develop a knowledge base that will allow self-reliance in coping with chronic illnesses. Care coordination is performed throughout the continuum of care from case referral to case closure and will involve direct member contact. Case planning will be thoroughly conducted to include determining and documenting specific objectives, goals and actions all designed to meet the member's identified needs. Plans should be time-specific and action oriented. The CCD Manager also delivers condition specific education in accordance with established best practice standards of care, and based on clinical practice guidelines.
