AllCare Health is seeking a Full-Time Care Coordination Supervisor who thrives in a team environment and has the ability to provide exceptional service and skills.
AllCare Health is a southern Oregon-based health organization, led by local physicians since 1994. In a progressive model of care, we combine medical, dental, and behavioral health services to provide Medicare, and Medicaid insurance coverage to our members. We also provide support to our providers through AllCare Independent Physician Association and AllCare e-Health Services

The Care Coordination Supervisor is responsible for providing program support to the Care Coordination Manager. The position supports the Care Coordination project working with high risk members who need assistance in understanding their health condition(s) and navigating health and social service systems. This position provides direct services to members through coordination of program referrals and supervision of care coordination staff.

Qualifications:
 Current RN licensure free of any restrictions of disciplinary action pending and able to legally and independently practice without the supervision of another licensed professional.
 Experience in health care required
 One to three years’ experience in program administration including grants management, community collaboration, contract management, supervision, and data management
 Two to four years’ experience working with underserved populations
 Demonstrated experience working with diverse populations in community settings
 Ability to prioritize heavy workloads and manage multiple tasks with different deadlines

Essential Duties:
 Selects, schedules, assigns, and evaluates staff, adjusting hours and shifts as necessary.
 Orients new staff and assures that training and education needs of staff are met.
 Coordinates department activities and the work assignments of staff members.
 Evaluates activities of department to ensure member care, staff relations, and efficiency of service.
 Directs the nursing assessment of all members.
 Develops, implements, and evaluates policies, goals, and objectives.
 Participates in the development and modifications of department programs.
 Participates in discharge planning, utilization review, case management, and quality assessment activities and analysis.
 Interprets and enforces department policies.
 Participates in appropriate department committees.
 Directs preparation and maintenance of members' clinical records.
