AllCare Health is seeking a Full-Time Social Support Care Coordinator who thrives in a team environment and has the ability to provide exceptional service and skills.

AllCare Health is a southern Oregon-based health organization, led by local physicians since 1994. In a progressive model of care, we combine medical, dental, and behavioral health services to provide Medicare, and Medicaid insurance coverage to our members. We also provide support to our providers through AllCare Independent Physician Association and AllCare e-Health Services.

The Social Support Care Coordinator position delivers evidence-based wellness promotion and self-care management solutions to Medicare and Medicaid members of all ages who have chronic diseases and/or conditions. The process shall include assessment, planning, implementation, coordination and request for services required to meet member’s health care needs.

Qualifications:

Current MSW, LCSW in the state where they work free of any restrictions of disciplinary action pending and able to legally and independently practice without the supervision of another licensed professional.
CCM certification preferred or successfully completing the certification within two years of employment
Three years clinical experience with preferred experience in conditions they will be managing.
At least 2 years previous experience to include: Case Management, Disease Management, Managed Care, or Homecare Coordination.
Essential Duties:

All care coordination to be performed throughout the continuum of care from case referral to case closure and may involve direct member contact.
Complete a thorough assessment that collects in-depth information about a Medicare dual eligible member’s unique situation and functioning in order to identify their individual needs. Information gathered may include direct client contact information and all other relevant sources. Based on identified needs develop a comprehensive case management plan.
Participate as an integral member of the Interdisciplinary Care Team when member’s individual needs are complex and require the skills and expertise of the ICT and case review approach.
Must possess an understanding of Medicare dual eligible eligibility requirements.
Must understand all aspects of the Special Needs Plan: Model of Care Program.
Must be knowledgeable in Medicare dual eligible benefits, AllCare OHP benefits, programs and be able to administer policies consistently and in accordance with the contracts.
AllCare offers a great working environment. Candidates offered a position must be able to successfully pass a drug test and background check. Skills testing may be required.
